          GYN PATH Services, Inc. relies on insurance 
          information provided by the referring physician who ordered your 
          laboratory tests. In some instances, this information may not be 
          sufficient for us to submit a claim to your insurance carrier for 
          reimbursement. Should you receive a bill from GYN PATH and have any 
          questions or concerns please contact us between the hours of 8 AM and 
          5 PM Monday through Friday at 915-755-8478 or toll free at 
          888-755-8478, and speak with one of our Patient Billing Account 
          Representatives. Please have your information available when calling. 
           
          GYN PATH understands how confusing the insurance industry has become 
          in the past few years. Additionally, with healthcare costs on the rise 
          you want your insurance to cover your healthcare expenses. We have 
          staff with many years of expertise in filing and appealing claims for 
          the maximum level of reimbursement. We will do everything we can to 
          assure your claim is submitted, processed correctly and finalized in a

          | Filing Primary Insurance | 
        
        
          If you are covered under Medicare or Medicaid please 
          contact us at 915-755-8478 and we will submit a claim to the 
          appropriate agency. Medicare and Medicaid have timely filing deadlines 
          for claims to be considered for reimbursement. Please contact us as 
          soon as possible with the requested information in order for the 
          services to be covered. 
           
          If you are covered under other insurance please contact us at 
          915-755-8478 and we will submit a claim to the appropriate carrier. 
          You may also fax a copy (front and back) of your insurance information 
          along with your invoice to 915-755-7331. Many insurance carriers have 
          filing deadlines in order for claims to be considered for 
          reimbursement. It is the patient’s responsibility to notify us of the 
          correct information in a timely manner or the balance may be your 
          responsibility. |

          | Filing Secondary Insurance | 
        
        
          GYN PATH Services does not automatically submit claims 
          to secondary insurance carriers. In order to submit a claim for 
          processing with your secondary insurance plan, you must forward a copy 
          of the invoice and a copy of your primary carrier’s Explanation of 
          Benefits to the claims address listed on your secondary 
          insurance card. 
           
          If you have Medicare as a secondary payer please forward a copy of 
          your primary insurance Explanation of Benefits along with your invoice 
          or you may contact us at 915-755-8478 and speak with our Patient 
          Billing Account Representatives. It is advised you contact your 
          primary carrier and let them know you have Medicare as a secondary 
          payer. Certain carriers will automatically forward the claim to 
          Medicare if they have the information on file.  |
